- Provide guidance and support on organizations available to patients, carers, and family members, offering beneficial personal services for various conditions such as miscarriage and SIDS, dementia, motor neuron diseases including Parkinson’s and muscular dystrophy, cancer, pain management, and more.
- Assistance with navigating Australian body donation programs and Organ Donation Australia
- Assistance in navigating private and government support options available through Hospice Care, Home Care, and Palliative Care, both personal and Guidance on NDIS eligibility requirements
- Providing continuous support and developing a customised plan for patients, caregivers, and family members throughout the end-of-life journey. This includes facilitating the return home for their final moments and organizing a personal vigil, fostering meaningful time for family and friends to come together.
